Print Lab Slip

To begin, click here to print a Digital Design Studio slip. The completed slip must contain the doctor's orders, license number, and signature.

Prepare Your Case

To ensure your stone models are not damaged during transit, please wrap and secure them. If you are sending multiple cases, please utilize same-sized boxes that are taped together in a secure fashion. Bundling multiple cases for shipping is preferred.

Request a Pick Up

If you are ready to request a pick up, please click here. We will provide you with shipping labels for incoming packages. Please specificy if a shipping label is needed and make sure your package is ready at the requested time.
